ALL club participants must be authorized by the office of Risk Management to drive other individuals outside of themselves on club-funded trips (including Common Adventure Trips)
Individuals wishing to drive only themselves DO NOT need to be authorized as drivers
Only ONE Driver Authorization Form is required to be submitted and approved per student – if you have Driver Authorization from another club or job on campus, you do not need to re-submit your paperwork.
Authorization status carries with student, or coach, for as long as they are registered as such in the University directory.
Proof of updated out-of-state and/or in-state driving record may be requested by administration at any time to maintain authorized driver status. Out-of-state drivers will be required to submit driver records after one year of initial approval
Read, acknowledge, and complete the Driver Authorization Agreement on the Stanford Alpine Club registration form.
Complete the Driver Authorization Form
Attach legible photocopy of driver’s license to Driver Authorization Form.
*Photocopy of driver’s license may also be included as a separate document
Indicate (check box) whether you have an out-of-state license OR have had your California Driver License for less than three years.
If either box is checked, you are responsible for obtaining your out-of-state, or in-state, driving record and submitting it with your form.

Indicate whether you are an: Employee, Temporary Employee (not eligible), Student, or Volunteer
Provide your signature
Input date of signature
Clearly print your name
Input your Driver License number
Input your Date of Birth
Leave Department Section BLANK.
Attach Driver Authorization Form and Driving Record, if required, in an email. For security purposes, encrypt the email by typing SECURE: in subject line (no text before or after necessary) and send to Assistant Director of Adventure Programs, Alyson Chun – achun2@stanford.edu
Staff will complete the Department Section and send to Office of Risk Management for approval.
Office of Risk Management will approve individual as an authorized driver for Adventure Clubs.
Allow up to seven (7) business days for approval
Staff will update authorized driver status on student’s registration profile. Student will receive a confirmation email.
